Abstract

Diabetes Mellitus type II is a type of diabetes mellitus in which the pancreas does not produce enough insulin so that it makes high blood sugar levels caused by the body being unable to respond to insulin. Complications that can occur are diabetic ulcers or impaired skin integrity. The purpose of writing this paper is to describe the nursing care of patients with type II diabetes mellitus with impaired skin integrity. The method used in the preparation of this paper is a descriptive method by approaching case studies with two subjects in the Work Unit of the Sukawati Public Health Unit I. Based on the study of research with a case study approach in Mr.IWS and Mrs. NMR found a gap between assessment and theory with no discovery. signs of minor symptoms such as pain, bleeding and hematoma in both patients, the nursing problem that is enforced is a disorder of skin integrity. The action taken is wound care, patient mobilization, and monitor patient's diits. After five days of nursing action there was an improvement in the condition. The results obtained are no signs of infection, showing an understanding of the wound process and preventing recurring injuries, as well as showing the wound healing process. Suggestions for patients to help their families be able to properly treat wounds at home once a day and prevent recurring injuries. Keywords: Diabetes, Skin Integrity Disorders, Nursing Care Diabetes Mellitus tipe II adalah jenis diabetes mellitus yang pankreasnya tidak menghasilkan insulin yang cukup sehingga membuat kadar gula darah menjadi tinggi yang disebabkan oleh tubuh yang tidak dapat merespon insulin. Komplikasi yang dapat terjadi adalah ulkus diabetika atau gangguan integritas kulit. Tujuan penulisan karya tulis ini adalah untuk menggambarkan asuhan keperawatan pasien diabetes mellitus tipe II dengan gangguan integritas kulit. Metode yang digunakan dalam penyusunan karya tulisan ini adalah metode deskriptif dengan pendekatkan studi kasus dengan dua subjek di Wilayah Kerja UPT Kesmas Sukawati I. Berdasarkan pengkajian penelitian dengan pendekatan studi kasus pada Tn.IWS dan Ny.NMR ditemukan kesenjangan antara pengkajian dan teori dengan tidak ditemukannya tanda gejala minor berupa nyeri, perdarahan dan hematoma pada kedua pasien, masalah keperawatan yang ditegakkan ialah gangguan integritas kulit. Tindakan yang dilakukan yaitu perawatan luka, mobilisasi pasien, dan pantau diit pasien. Setelah lima hari diberikan tindakan keperawatan terjadi peningkatan kondisi. Hasil yang didapatkan yaitu tidak terjadi tanda-tanda infeksi, menunjukkan pemahaman tentang proses luka dan mencegah terjadinya cidera berulang, serta menunjukkan proses penyembuhan luka. Saran agar pasien dibantu keluarganya mampu melakukan perawatan luka secara tepat dirumah satu kali sehari dan mencegah terjadi cidera berulang.
