Abstract

DESCRIPTION OF NURSING DIABETES MELITUS TYPE II IN FULFILLING THE HEALTH MANAGEMENT IMPROVEMENT OF HEALTH MANAGEMENT IN PUSKESMAS I DENPASAR BARAT IN 2020 ABSTRACT Diabetes mellitus is a serious chronic disease that occurs because the pancreas is unable to produce enough insulin (Khairani, 2018). The incidence of Type II Diabetes Mellitus in 2018 in Denpasar is 9,123. The purpose of the study is as one of the graduation requirements. This research uses descriptive research with data collection techniques of patient documentation study. The results of a preliminary study obtained data on DM visits at Puskesmas 1 Denpasar Barat in 2018 were 1620 people and 2019 as many as 1726 people. The results of the study showed that the two patients were not able to explain the difficulties in carrying out the treatment program marked by the patient still hesitant and embarrassed in explaining the difficulties in the treatment program, then obtained patient data 1 with GDS 220 MG / DL, and patient 2 with GDS 207 MG / DL. The diagnosis made is the readiness to improve health management. The interventions given for 30 minutes at each meeting took the form of health education at the first meeting, physical exercise education at the second meeting, and promotion of health effort behavior at the last meeting. Implementation of nursing is done 3 times. Nursing evaluation uses SOAP documentation. The evaluation obtained was that the two patients said the constraint was lazy to take medication, then both patients were able to describe the constraints experienced well, and there was a decrease in the GDS of the two patients, namely patients 1 with 200, and patients 2 with GDS 205. Conclusions from the literature review results this resulted in the results of both clients being able to explain their difficulties and experiencing a decrease in GDS. Suggestions for patients are to continue to do physical activities routinely, apply the education that has been given and always do healthy behavior. Keywords: nursing care, diabetes mellitus, health management  GAMBARAN ASUHAN KEPERAWATAN DIABETES MELITUS TIPE II DALAM PEMENUHAN KESIAPAN PENINGKATAN MANAJEMEN KESEHATAN DI PUSKESMAS I DENPASAR BARAT TAHUN 2020 ABSTRAK Diabetes Melitus merupakan penyakit kronis serius yang terjadi karena pancreas tidak mampu menghasilkan cukup insulin (Khairani, 2018). Angka kejadian Diabetes Melitus Tipe II pada tahun 2018 di Denpasar adalah 9.123. Tujuan penelitian adalah sebagai salah satu syarat kelulusan. Penelitian ini menggunakan jenis penelitian deskriptif dengan teknik pengumpulan data studi dokumentasi dokumen pasien . Hasil studi pendahuluan didapatkan data kunjungan DM di Puskesmas 1 Denpasar Barat pada tahun 2018 adalah 1620 orang dan 2019 sebanyak 1726 orang. Hasil pengkajian didapatkan kedua pasien belum mampu menjelaskan kesulitan dalam melakukan program perawatan ditandai dengan pasien masih ragu dan malu dalam menjelaskan kesulitannya dalam program perawatan, kemudian didapatkan data pasien 1 dengan GDS 220 MG/DL, dan pasien 2 dengan GDS 207 MG/DL. Diagnosis yang ditegakkan adalah kesiapan peningkatan manajemen kesehatan. Intervensi yang diberikan selama 30 menit setiap pertemuan berupa edukasi kesehatan pada pertemuan pertama, edukasi latihan fisik pada pertemuan kedua, dan promosi perilaku upaya kesehatan pada pertemuan terakhir. Implementasi keperawatan yang dilakukan sebanyak 3 kali pertemuan. Evaluasi keperawatan menggunakan pendokumentasian SOAP. Evaluasi yang didapatkan adalah kedua pasien mengatakan kendalanya adalah malas minum obat, kemudian kedua pasien mampu menjabarkan kendala yang dialami dengan baik, serta terjadi penurunan GDS kedua pasien yaitu pasien 1 dengan 200, dan pasien 2 dengan GDS 205. Simpulan dari hasil review literatur karya tulis ini diperoleh hasil kedua klien mampu menjelaskan kesulitannya serta mengalami penurunan GDS. Saran untuk pasien adalah tetap melakukan aktifitas fisik secara rutin, menerapkan edukasi yang telah diberikan serta selalu melakukan perilaku sehat.
