Abstract

COPD is a disease characterized by limited air flow, is progressive and is associated with an abnormal lung inflammatory response to harmful particles or gases, causing Goblet cells and sub-mucosal glands to work hard continuesly, resulting in hypertrophy and hyperplasi. Factors that inhibit mucociliary clearance are due to goblet cell poliferation and the replacement of ciliated epithelium with non-ciliated epithelium therefore causing a large accumulation of mucus that is difficult to remove from the airways. Causing the problem of ineffective airway clearance. This study aims to determine of nursing care in COPD patients with an ineffective airway clearance in the Cendrawasih Room of Wangaya Hospital in 2020 through the Polytechnic of Health Denpasar Repository. This type of research is a descriptive study with a case study design of two COPD patient documents by observing and comparing the two COPD patient documents as well as the theory used. The results showed that there were similarities in the two documents of COPD patients with airway clearance that were ineffective in documenting nursing care, but there were differences with the theories that the researchers used to reference with reference theories in the room, namely in the assessment, diagnosis, plan, implementation and evaluation of nursing. Keywords : Nursing care, COPD, Ineffective Airway Clearance PPOK merupakan penyakit yang dikarakteristikkan oleh keterbatasan aliran udara yang menetap, bersifat progresif dan dikaitkan dengan adanya respons inflamasi paru yang abnormal terhadap partikel atau gas berbahaya, sehingga menyebabkan sel-sel Goblet dan kelenjar sub-mukosa terus menerus bekerja keras, maka terjadi hipertrofi dan hiperplasi. Faktor yang menghambat pembersihan mukosiliaris adalah karena adanya poliferasi sel goblet dan pergantian epitel yang bersilia dengan yang tidak bersilia oleh karena itu menyebabkan penumpukan mucus dalam jumlah besar yang sulit dikeluarkan dari saluran napas. Sehingga menimbulkan masalah bersihan jalan napas tidak efektif. Penelitian ini bertujuan untuk mengetahui gambaran asuhan keperawatan pada pasien PPOK dengan bersihan jalan napas tidak efektif di Ruang Cendrawasih RSUD Wangaya tahun 2020 melalui Repository Poltekkes Denpasar. Jenis penelitian yang digunakan adalah penelitian deskriptif dengan rancangan studi kasus terhadap dua dokumen pasien PPOK dengan mengamati dan membandingkan kedua dokumen pasien PPOK serta dengan teori yang digunakan. Hasil penelitian menunjukkan bahwa terdapat kesamaan dari dua dokumen pasien PPOK dengan bersihan jalan napas tidak efektif dalam pendokumentasian asuhan keperawatan, namun terdapat perbedaan dengan teori yang menjadi acuan peneliti dengan teori acuan di ruangan yaitu pada pengkajian, diagnosa, rencana, implementasi dan evaluasi keperawatan. Perbedaan terjadi karena teori yang digunakan di rumah sakit berbeda dengan peneliti. Kata kunci : Asuhan Keperawatan, PPOK, Bersihan Jalan Napas Tidak Efektif
