Abstract

Penyakit gagal ginjal kronis adalah suatu keadaan klinis yang ditandai dengan menurunnya fungsi ginjal secara progresif dan bersifat ireversibel. Pasien gagal ginjal kronis memerlukan terapi pengganti ginjal berupa hemodialisis atau transplantasi ginjal. Pasien hemodialisis rentan mengalami malnutrisi yang diakibatkan oleh asupan nutrisi yang dibatasi dan asidosis metabolik, peningkatan hormon leptin yang berujung pada berkurangnya nafsu makan, pengaruh obat-obatan, pengambilan sampel darah yang berulang, dan pelepasan asam amino melalui dialisat sehingga terjadi penurunan sintesis protein. Tujuan penelitian ini adalah untuk mengetahui hubungan antara tingkat konsumsi protein dan lama hemodialisis dengan status gizi pasien gagal ginjal kronis. Jenis penelitian observasional dengan desain cross sectional. Sampel penelitian sejumlah 34 orang yang ditentukan secara consecutive sampling. Data yang dikumpulkan berupa karakteristik sampel, tingkat konsumsi protein, lama hemodialisis, dan status gizi melalui metode wawancara, anamnesa dan pemeriksaan fisik, serta mencatat data rekam medik. Berdasarkan kriteria tingkat konsumsi protein diperoleh hasil sebanyak 31 orang (91,2%) yang tidak sesuai dengan kebutuhan protein perharinya. Sebagian besar sampel yang menjalani hemodialisis sudah > 3 tahun yaitu sebanyak 15 orang (44,1%). Status gizi sampel terbanyak dalam kategori malnutrisi ringan sampai sedang dengan indikator status gizi dalam rentang skor 14-23 yaitu sebanyak 19 orang (55,9%). Hasil uji korelasi Rank Spearman didapatkan tidak ada hubungan (nilai p = 0,70) antara tingkat konsumsi protein dengan status gizi, dengan (nilai r = - 0,06) dan tidak ada hubungan (nilai p = 0,08) antara lama hemodialisis dengan status gizi dengan (nilai r = 0,29). Kesimpulan dari penelitian ini adalah tidak ada hubungan antara tingkat konsumsi protein dan lama hemodialisis dengan status gizi pasien gagal ginjal kronis di RSUD Wangaya Denpasar. Patients with chronic kidney failure require kidney replacement therapy in the form of hemodialysis or kidney transplantation. Hemodialysis patients are susceptible to malnutrition due to restricted nutrient intake and metabolic acidosis, an increase in leptin which leads to decreased appetite, the influence of drugs, repeated blood sampling, and release of amino acids through dialysate resulting in decreased protein synthesis. The purpose of this study was to determine the relationship between the level of protein consumption and length of hemodialysis with the nutritional status of patients with chronic kidney failure. This type of observational research with cross sectional design. The research sample of 34 people who were determined by consecutive sampling. Data collected in the form of sample characteristics, level of protein consumption, length of hemodialysis, and nutritional status were obtained through interview methods, anamnesa and physical examination, as well as recording medical record data. Based on the criteria for the level of protein consumption the results obtained were 31 people (91.2%) who were not in accordance with their daily protein needs. Most of the samples undergoing hemodialysis were > 3 years, as many as 15 people (44.1%). The most nutritional status of the sample is in the category of mild to moderate malnutrition with an indicator of nutritional status in the range of scores from 14-23 as many as 19 people (55.9%). Spearman Rank correlation test results found no relationship (p value = 0.70) between the level of protein consumption with the nutritional status, with (r value = -0.06) and no relationship (p value = 0.08) between length of hemodialysis with the nutritional status (r = 0.29). The conclusion of this study is that there is no relationship between the level of protein consumption and length of hemodialysis with the nutritional status of patients with chronic kidney failure at Wangaya Regional General Hospital Denpasar.
