Abstract
GAMBARAN ASUHAN KEPERAWATAN PADA PASIEN STROKE HEMORAGIK DENGAN BERSIHAN JALAN NAFAS TIDAK EFEKTIF DI RUANG OLEG RSD MANGUSADA BADUNG TAHUN 2019 ABSTRAK Bersihan jalan nafas tidak efektif adalah suatu keadaan dimana individu tidak mampu membersihkan secret untuk mempertahankan kepatenan jalan nafas. Tujuan umum dari penelitian ini adalah untuk mengetahui gambaran Asuhan Keperawatan Pada Pasien Stroke Hemoragik dengan Bersihan Jalan Nafas Tidak Efektif di Ruang Oleg RSD Mangusada Badung dengan metode dokumentasi dokumen pasien yang dilaksanakan pada tanggal 20-27 April 2019. Dengan hasil kedua pasien 1(Tn.S) 65 tahun dan pasien 2(Tn.K) 68 tahun. Pengkajian pada dokumen subyek pertama dan kedua data subjektif dan data obyektif hasilnya sama. Setelah dibandingkan terdapat perbedaan antara teori dan hasil pengkajian, pada diagnosa keperawatan yang ditegakkan pada kedua subyek terdapat perbedaan dengan teori yaitu belum menggunakan format PES melainkan hanya P, pada perencanaan yang dibuat teradapat beberapa perbedaan dengan teori karena Pada dokumen kedua subjek masih menggunakan NIC & NOC, namun pada teori menggunakan SIKI & SLKI. Implementasi keperawatan dilakukan sesuai dengan rencana tindakan yang telah ditetapkan, namun ada beberapa intervensi yang tidak di implementasikan. Hasil evaluasi yang didapatkan pada dokumen kedua subyek menggunakan SOAP. Dengan hal tersebut diharapkan pihak RSD Badung dalam pemberian asuhan keperawatan di Ruang Oleg lebih memperhatikan acuan teori terbaru sehingga dalam memberikan asuhan keperawatan dapat dilakukan secara maksimal. Kata Kunci : Asuhan keperawatan, bersihan jalan nafas tidak efektif, stroke hemoragik NURSING CARE DESCRIPTION OF NURSING CARE IN PATIENTS STROKE HAEMORAGIC WITH UNCOMMITED ROAD BREATHING IN OLEG WARD OF MANGUSADA HOSPITAL 2019 ABSTRCT Ineffective airway clearing is a condition where the individual is unable to clear the secret to maintain the airway's patency. The general objective of this study was to find out the description of Nursing Care in Hemorrhagic Stroke Patients with Breathing Pathways Not Effective in the Oleg Room of Mangusada Badung Hospital with the method of documenting patient documents carried out on April 20-27 2019. With the results of both patients 1 (Mr. S) 65 years and patient 2 (Mr. K) 68 years. Assessment of the first and second subject documents subjective data sssand the objective data results are the same. After comparing there are differences between the theory and the results of the study, nursing diagnoses that are enforced on both subjects have differences with the theory, that is, they have not used the PES format but only P, in the planning there are some differences with the theory because the two documents still use NICs & NOCs. , but on the theory of using SIKI & SLKI. Nursing implementation is carried out in accordance with the established action plan, but there are several interventions that are not implemented. The evaluation results obtained on the documents of the two subjects used SOAP.
