Abstract

NURSING CARE FOR MRS. W WITH INEFFECTIVE AIRWAY CLEARANCE DUE TO PULMONARY TBC IN THE BANGLI COMMUNITY HEALTH CENTER WORKING AREA IN 2026 ABSTRACT Tuberculosis (TB) is a contagious infectious disease caused by Mycobacterium tuberculosis that remains a major global and national health threat. The most common nursing problem encountered is ineffective airway clearance due to the accumulation of thick secretions in the respiratory tract. This case report aims to provide comprehensive nursing care for Mrs. W, focusing on ineffective airway clearance caused by pulmonary TB within the Bangli Health Center's working area. This study employs a nursing process approach, including assessment, diagnosis, intervention, implementation, and evaluation. The primary intervention consisted of effective coughing exercises and semi-Fowler positioning administered over four days, with 30-minute sessions per visit. Initial assessment revealed the patient experienced shortness of breath (RR 26 bpm), ineffective cough, and rhonchi breath sounds. Following the consistent implementation of effective coughing exercises, the patient was able to expectorate secretions independently, respiratory distress decreased, and the respiratory rate improved to 20 bpm. The application of effective coughing exercises is proven to be effective in improving airway clearance in pulmonary TB patients. Patients and families are encouraged to continue these techniques independently to support the healing process. Keyword : Airway Clearance, Effective Coughing  ASUHAN KEPERAWATAN PADA NY.W DENGAN BERSIHAN JALAN NAFAS TIDAK EFEKTIF AKIBAT TBC PARU DI WILAYAH KERJA PUSKESMAS BANGLI TAHUN 2026 ABSTRAK Tuberkulosis (TBC) merupakan penyakit infeksi menular akibat Mycobacterium tuberculosis yang menjadi ancaman kesehatan global dan nasional. Masalah keperawatan yang paling sering muncul adalah bersihan jalan napas tidak efektif akibat akumulasi sekret kental di saluran pernapasan. Tujuan: Melaksanakan asuhan keperawatan komprehensif pada Ny. W dengan masalah bersihan jalan napas tidak efektif akibat TBC paru di wilayah kerja Puskesmas Bangli. Metode: Laporan kasus ini menggunakan pendekatan proses keperawatan yang meliputi pengkajian, diagnosis, intervensi, implementasi, dan evaluasi. Intervensi utama yang diberikan adalah latihan batuk efektif dan pengaturan posisi semi-fowler selama 4 hari dengan durasi 30 menit per sesi. Hasil: Pengkajian awal menunjukkan pasien mengalami sesak napas (RR 26x/menit), batuk tidak efektif, dan suara napas ronkhi. Setelah diberikan intervensi latihan batuk efektif secara rutin, pasien mampu mengeluarkan sekret secara mandiri, sesak napas berkurang, dan frekuensi napas membaik menjadi 20x/menit. Simpulan: Penerapan latihan batuk efektif terbukti efektif dalam meningkatkan bersihan jalan napas pada pasien TBC paru. Pasien dan keluarga diharapkan dapat melanjutkan teknik ini secara mandiri untuk mendukung proses penyembuhan. Kata kunci: bersihan jalan napas, batuk efektif
