Abstract
NURSING CARE FOR MRS. K WITH IMPAIRED SKIN AND TISSUE INTEGRITY DUE TO TYPE 2 DIABETES MELLITUS IN THE LEGONG ROOM AT MANGUSADA REGIONAL GENERAL HOSPITAL IN 2026 ABSTRACT Type 2 diabetes mellitus is a chronic metabolic disease characterized by hyperglycemia. Common complaints experienced by patients with type 2 diabetes mellitus include polyuria, polydipsia, and polyphagia. One of the most common complications is Diabetic Foot Ulcer (DFU), which can lead to impaired skin and tissue integrity. This study aimed to determine the nursing care provided to Mrs. K with impaired skin/tissue integrity due to type 2 diabetes mellitus in the Legong Ward of Mangusada Regional Hospital in 2026. This study used a descriptive case study method with Mrs. K, a 67-year-old woman, as the subject. Laboratory examination results showed a Random Blood Glucose (RBG) level of 560 mg/dL. The nursing diagnosis established was impaired skin and tissue integrity related to peripheral neuropathy. Nursing interventions focused primarily on wound care. After five days of nursing treatment, evaluation findings indicated improvement in the patient’s condition. Subjectively, the patient reported reduced pain in the right foot. Objectively, a wound was still found on the dorsum pedis dextra with a diameter of approximately 15–20 cm; however, there was a decrease in the pain scale to 2 from 4 (0–10), as well as a reduction in bleeding, redness, and hematoma. It can be concluded that the nursing problem of impaired skin/tissue integrity was partially resolved, with signs and symptoms improving by 80%. K DENGAN GANGGUAN INTEGRITAS KULIT/JARINGAN AKIBAT DIABETES MELLITUS TIPE 2 DI RUANG LEGONG RSD MANGUSADA TAHUN 2026 ABSTRAK Diabetes melitus tipe 2 merupakan penyakit metabolik kronis yang ditandai dengan hiperglikemia. Keluhan yang umum dialami pasien diabetes mellittus tipe 2 meliputi polyuria, polydipsia dan polifagia. Salah satu komplikasi yang paling umum terjadi yaitu Ulkus kaki Diabetik (UKD) yang menyebabkan terjadinya gangguan integritas kulit dan jaringan. Penelitian ini bertujuan untuk mengetahui asuhan keperawatan pada Ny. K dengan gangguan integritas Kulit/Jaringan akibat diabetes melitus tipe 2 di Ruang Legong RSD Mangusada Tahun 2026. Penelitian ini menggunakan metode studi kasus deskriptif dengan subjek Ny. K, seorang Perempuan berusia 67 tahun. Hasil pemeriksaan labolatorium pasien menunjukkan kadar Gula Darah Sewaktu (GDS) yaitu 560 mg/dL. Diagnosis keperawatan yang ditegakkan adalah gangguan integritas Kulit/Jaringan akibat neuropati perifer. Intervensi keperawatan yang diberikan berfokus pada perawatan luka dengan metode modern dressing. Setelah lima hari perawatan hasil evaluasi menunjukkan adanya perbaikan kondisi pasien. Secara subjektif, pasien melaporkan penurunan nyeri pada bagian luka. Secara objektif, masih ditemukan luka pada dorsum pedis dextra dengan diameter ±15–20 cm, namun terjadi penurunan skala nyeri menjadi 2 dari 4 (0-10), serta penurunan perdarahan, kemerahan, dan hematoma. Dapat disimpulkan bahwa masalah keperawatan gangguan integritas kulit/jaringan teratasi sebagian, tanda dan gejala teratasi 80%.
