Abstract

THE RELATIONSHIP OF ANEMIA AND GLOMERULUS FILTRATION RATE (eGFR) IN PATIENTS WITH TYPE 2 DIABETES MELLITUS AND CHRONIC KIDNEY DISEASE (CKD) ABSTRACT Background: Anemia is a common complication in patients with type 2 diabetes mellitus (T2DM) and chronic kidney disease (CKD). However, its relationship to decreased kidney function, as measured by glomerular filtration rate (eGFR), requires further study. This study aimed to determine the relationship between anemia and eGFR values in patients with T2DM and CKD stage 3 or higher at the State General Hospital. Aim: The study used an observational analytical design with a cross-sectional approach based on medical record data. Methods: The sample consisted of 101 patients who met the inclusion and exclusion criteria. Bivariate analysis was performed using Spearman correlation to asses the strength and direction of the relationship between eGFR and hemoglobin levels. Results: The results showed a prevalence of anemia of 55.4%. The Spearman correlation test yielded a correlation coefficient of r = 0.44 (p < 0.001), indicating a moderate positive relationship between eGFR and hemoglobin levels. The prevalence of anemia increased from 42.9% in CKD stage 3a to 93.5% in stage 5, accompanied by a decrease in mean hemoglobin from 12.2 g/dL to 7.6 g/dL. Conclusion: It was concluded that there is a significant relationship between anemia and glomerular filtration rate in patients with type 2 diabetes mellitus (T2DM) with CKD stage 3 or higher. Regular hemoglobin monitoring is highly recommended, especially in patients with eGFR below 45 ml/min/1.73 m². Tujuan: Penelitian ini bertujuan untuk mengetahui hubungan antara anemia dengan nilai eGFR pada pasien DMT2 dengan CKD stadium 3 atau lebih di Rumah Sakit Umum Negara. Penelitian menggunakan desain analitik observasional dengan pendekatan cross-sectional berbasis data rekam medis. Sampel berjumlah 101 pasien yang memenuhi kriteria inklusi dan eksklusi. Analisis bivariat dilakukan menggunakan korelasi Spearman untuk menilai keuatkan dan arh hubungan antara eGFR dan kadar hemoglobin. Hasil: penelitian ini menunjukkan prevalensi anemia sebesar 55,4%. Uji korelasi Spearman menghasilkan koefisien korelasi r = 0,44 (p < 0,001), mengindikasikan hubungan positif cukup (moderate) antara eGFR dan kadar hemoglobin. Prevalensi anemia meningkat dari 42,9% pada stadium CKD 3a menjadi 93,5% pada stadium 5, disertai penurunan rerata hemoglobin dari 12,2 g/dL menjadi 7,6 g/dL. Kesimpulan: hasil ini terdapat hubungan yang bermakna antara anemia dan laju filtrasi glomerulus pada pasien DMT2 dengan CKD stadium 3 atau lebih. Pemantauan hemoglobin secara berkala sangat direkomendasikan, terutama pada pasien dengan eGFR di bawah 45 ml/mnt/1,73 m². Kata Kunci: anemia, laju filtrasi glomerulus, diabetes melitus tipe 2
