Abstract

GAMBARAN ASUHAN KEPERAWATAN PADA PASIEN STROKE NON HEMORAGIK DENGANGANGGUAN MOBILITAS FISIK DI RUANG SAHADEWA RSUD SANJIWANI GIANYAR ABSTRAK Nekrosis pada area broadman empat dan enam pada stroke non hemoragik menyebabkan gangguan mobilitas fisik. Tujuan umum pada penelitian ini adalah mengetahui gambaran asuhan keperawatan pada pasien stroke non hemoragik dengan gangguan mobilitas fisik di ruang Sahadewa RSUD Sanjiwani Gianyar. Penelitian ini dilakukan dengan menggunakan metode deskriptif dan teknik pengumpulan data dengan studi dokumentasi melalui dua dokumen pasien yang dilaksanakan pada bulan April 2019. Hasil penelitian yang didapatkan dari kedua dokumen subyek yaitu tidak ditemukan data subjektif pada pengkajian, data objektif dokumen pasien pertama yaitu kekuatan otot menurun dan kemampuan mobilitas dibantu orang lain, sedangkan data objekti dokumen dua yaitu kekuatan otot menurun, kemampuan mobilitas dibantu dengan alat dan orang lain. Diagnosa keperawatan yang ditegakkan pada kedua dokumen subyek adalah kerusakan mobilitas fisik berhubungan dengan gangguan neurologis. Pada bagian intervensi keperawatan dirumuskan berdasarkan Nursing Interventions Classification (NIC) dan Nursing Outcome Classification (NOC). Pada bagian implementasi dilakukan sesuai dengan intervensi namun ada beberapa yang tidak didokumentasikan, tahap evaluasi keperawatan menggunakan SOAP. Hasil penelitian berbeda dengan teori acuan peneliti. Hal tersebut dikarenakan rumah sakit masih menggunakan teori Nanda NIC-NOC sedangkan peneliti menggunakan acuan teori SDKI, SLKI dan SIKI. Kata kunci : asuhan keperawatan, gangguan mobilitas fisik, stroke non hemoragik DESCRIPTION OF NURSING CARE FOR STROKE NON HEMORRHAGIC PATIENTS WITH DISORDER PHYSICAL MOBILITY IN SAHADEWA WARD OF SANJIWANI GIANYAR HOSPITAL ABSTRACT Necrosis in broadman areas four and six in stroke non hemorrhagic causes disorder physical mobility. The general objective of this research was to describe the description nursing care of stroke non hemorrhagic patients with disorder physical mobility in the Sahadewa ward of Sanjiwani Gianyar Hospital. This research was conducted by using descriptive methods and data collection techniques with documentation studies through two patient documents carried out in April 2019. The results of the research obtained from the two subject documents were that there were no subjective data on the assessment, the first objective document of patient data was decreased muscle strength and mobility ability assisted by others, while the objective data of document two was decreased muscle strength, mobility ability aided by tools and others. Nursing diagnoses that are enforced on both subject documents are damage physical mobility associated with neurological disorders. In the nursing intervention section formulated based on Nursing Interventions Classification (NIC) and Nursing Outcome Classification (NOC). The implementation section is carried out in accordance with the intervention but some implementation are not documented, nursing evaluation stages using SOAP. The results of the study are different from the researchers' reference theory. This is because the hospital still uses the Nanda NIC-NOC theory while the researcher uses the theoretical reference of SDKI, SLKI and SIKI.
