Abstract

COPD is a chronic lung inflammation due to exposure to harmful gases and smoking habits that cause obstruction in the airways. According to WHO, COPD is the fourth leading cause of death in the world and is one of the main health problems in developing countries. This case report aims to describe the implementation of nursing care for Mrs. T with ineffective airway clearance due to COPD at Klungkung Regional Hospital year 2026. The method used is descriptive with assessment, diagnosis, intervention, implementation, and evaluation of nursing. Subjective data showed that the patient complained of shortness of breath, ineffective cough with excessive phlegm, and difficulty speaking. Objective data found were wheezing breath sounds, tachypnea breathing pattern, respiratory rate 28 x / minute, BP: 120/80 mmHg, N: 85x / minute, S: 36.6o C, SPO2: 97%. The nursing problem found was ineffective airway clearance. Interventions provided included effective coughing exercises, airway management, respiratory monitoring, and collaboration in administering nebulizers and oxygen therapy. The evaluation results showed improvement in the patient's condition, with reduced shortness of breath, more effective coughing, improved breathing pattern, wheezing (-), and a respiratory rate of 20 breaths/minute. Keywords: Nursing care, Ineffective airway clearance, COPD PPOK adalah peradangan paru kronis akibat paparan gas berbahaya dan kebiasaan merokok yang menyebabkan penyumbatan pada saluran napas. Menurut WHO, PPOK merupakan penyebab kematian keempat di dunia dan menjadi salah satu masalah kesehatan utama di negara berkembang. Laporan kasus ini bertujuan untuk menggambarkan pelaksanaan asuhan keperawatan pada Ny. T dengan bersihan jalan napas tidak efektif akibat PPOK di RSUD Klungkung tahun 2026. Metode yang digunakan adalah deskriptif dengan pengkajian, diagnosis, intervensi, implementasi, dan evaluasi keperawatan. Data subjektif menunjukkan pasien mengeluh sesak napas, batuk tidak efektif dengan dahak berlebihan, serta kesulitan berbicara. Data objektif yang ditemukan yaitu suara napas wheezing, pola napas takipnea, frekuensi napas 28 x/menit, TD: 120/80 mmHg, N: 85x/menit, S: 36,6o C, SPO2: 97% Masalah keperawatan yang ditemukan adalah bersihan jalan napas tidak efektif. Intervensi yang diberikan meliputi latihan batuk efektif, manajemen jalan napas, pemantauan respirasi, serta kolaborasi pemberian nebulizer dan terapi oksigen. Hasil evaluasi menunjukkan perbaikan kondisi pasien dengan sesak berkurang, batuk lebih efektif, pola nafas membaik, wheezing (-), frekuensi nafas 20 x/menit.
