Abstract

Midwifery Care on Mrs. “GW” 21 Years Old Multigravida from 16 Weeks 2 Days Pregnancy until 42 Days Postpartum Case Take Placed on Work Area of Puskesmas Banjarangkan 1I 2026 ABSTRACT Maternal Mortality Rate (MMR) and Infant Mortality Rate (IMR) are indicators that can describe the welfare of the community in a country. Continuity of care for mothers “GW” aims to determine the results of implementing midwifery care according to standards in a comprehensive and continuous manner and combined with complementary care to obtain maximum service results and increase comfort and minimize complaints. This study uses a case study with primary data and secondary data through interviews, examination, observation and documentation. Continuity of care for mothers from September 2025 to March 2026 in the working area of UPTD Puskesmas Banjarangkan II. The development of mother's pregnancy is physiological. Mother gave birth vaginally without complications on 38 weeks of pregnancy. The first stage lasts for threes hours from the opening of four to complete, the second stage lasts for 15 minutes, the third stage lasts for 5 minutes and the monitoring of the fourth stage is within normal limits. The baby was born immediately crying, active muscle tone, and birth weight 3400 grams. The process of uterine involution, lochea expenditure and lactation during the puerperium took place normally. Therefore, it is important to provide standardized midwifery care in an effort to monitor and detect complications and complications in the process of pregnancy, childbirth, postpartum and infant. In addition, the application of complementary care is also very beneficial in reducing maternal complaints and increasing the mothers sense of comfort in dealing with the process of pregnancy to birth. Keywords: continuity of care, pregnancy, childbirth, post partum, infant ASUHAN KEBIDANAN PADA IBU “GW” UMUR 21 TAHUN MULTIGRAVIDA DARI UMUR KEHAMILAN 16 MINGGU 2 HARI SAMPAI 42 HARI MASA NIFAS Asuhan Kasus dilakukan di Wilayah Kerja Unit Pelaksana Teknis Daerah Puskesmas Banjarangkan II Tahun 2026 ABSTRAK Angka Kematian Ibu (AKI) dan Angka Kematian Bayi (AKB) merupakan salah satu indikator yang dapat menggambarkan kesejahteraan masyarakat di suatu negara. Asuhan berkelanjutan atau Continuity of care ibu “GW” bertujuan untuk mengetahui hasil penerapan asuhan kebidanan sesuai standar secara komprehensif dan berkesinambungan serta dikombinasikan dengan asuhan komplementer untuk mendapatkan hasil pelayanan yang lebih maksimal dan meningkatkan rasa nyaman dan meminimalkan keluhan. Studi ini menggunakan studi kasus dengan teknik pengumpulan data primer dan data sekunder melalui wawancara, pemeriksaan, observasi serta dokumentasi. Asuhan diberikan dari bulan September 2025 sampai Maret 2026 di wilayah kerja UPTD Puskesmas Banjarangkan II. Perkembangan kehamilan ibu “GW” berjalan fisiologis. Ibu bersalin pervaginam tanpa komplikasi pada usia kehamilan 38 minggu. Kala I berlangsung selama tiga jam dari pembukaan empat sampai lengkap, kala II berlangsung selama 15 menit, kala III berlangsung selama 5 menit dan pemantauan kala IV dalam batas normal. Bayi lahir segera menangis, tonus otot aktif, dan berat lahir 3400 gram. Proses involusi uterus, pengeluaran lochea dan laktasi pada masa nifas berlangsung normal. Oleh karena itu, penting memberikan asuhan kebidanan sesuai standar sebagai upaya untuk memantau dan mendeteksi penyulit dan komplikasi pada proses kehamilan, persalinan, nifas dan bayi. Selain itu, penerapan asuhan komplementer juga sangat memberikan manfaat untuk mengurangi keluhan ibu dan menambah rasa nyaman ibu dalam menghadapi proses kehamilan sampai kelahiran.
