Abstract

ABSTRACT Continuity of midwifery care is an effort to improve the quality of maternal and neonatal health and to prevent complications. The care was provided to Mrs. “CJ” from 24 weeks and 4 days of gestation until 42 days postpartum and during the newborn period. Data were collected through interviews, examinations, observations, and documentation. The care was conducted from November 2025 to March 2026. During pregnancy, standard 12T antenatal care was implemented with a minimum of eight ANC visits, supported by complementary therapies including prenatal yoga and herbal remedies (ginger drink). Labor progressed physiologically according to standards. The first stage of labor lasted 3 hours and 25 minutes with complementary use of a birth ball and effleurage massage. The second stage lasted 40 minutes according to the 60 steps of Normal Delivery Care (APN), while the third stage lasted 4 minutes with active management. Monitoring during the fourth stage remained within normal limits. The baby was born crying strongly, actively moving, with a birth weight of 3100 grams, and early initiation of breastfeeding (IMD) was successfully achieved within the first hour. During the postpartum period, four standard postpartum visits were conducted: KF 1 at 6 hours postpartum, KF 2 at 6 days postpartum, KF 3 at 25 days postpartum, and KF 4 at 42 days postpartum. Monitoring results showed normal uterine involution, lochia discharge, and lactation, supported by complementary oxytocin massage and Kegel exercises. For the newborn, standard neonatal care was provided through KN 1 within the first 12 hours and blood sampling for newborn screening (SHK) at 48 hours after birth. KN 2 was conducted on day 6 and KN 3 on day 25. Neonatal growth and development up to 42 days progressed physiologically with complementary baby massage therapy. Overall, continuity of midwifery care was carried out physiologically and according to standards as an effort to monitor and detect complications early in both mother and baby. Asuhan diberikan pada ibu “CJ” umur kehamilan 24 minggu 4 hari sampai 42 hari masa nifas dan bayi baru lahir. Teknik pengumpulan data melalui wawancara, pemeriksaan, observasi, dokumentasi. Asuhan diberikan bulan November 2025 sampai Maret 2026. Pada kehamilan penerapan 12T sesuai standar dan pemeriksaan ANC minimal 8 kali kunjungan dengan komplementer Yoga prenatal dan Ramuan herbal (minuman jahe). Pada persalinan berjalan fisiologis sesuai standar. Kala I berlangsung 3 jam 25 menit, dengan komplementer birthball dan massage effleurage. Kala II berlangsung 40 menit sesuai 60 langkah APN, kala III berlangsung 4 menit dengan manajemen aktif kala III dan pemantauan kala IV dalam batas normal. Bayi lahir menangis kuat, gerak aktif, BB lahir 3100 gram, dilakukan IMD dan berhasil pada 1 jam pertama. Pada masa nifas dilakukan empat kali kunjungan sesuai standar, yaitu KF 1 pada 6 jam postpartum, KF 2 pada 6 hari postpartum, KF 3 pada 25 hari postpartum, dan KF 4 pada 42 hari postpartum. Hasil pemantauan menunjukkan involusi uterus, pengeluaran lochea, dan laktasi berlangsung normal dengan terapi komplementer berupa pijat oksitosin dan senam kegel. Pada bayi diberikan sesuai asuhan kebidanan sesuai standar melalui kunjungan KN 1 pada 12 jam pertama dan pada 48 jam setelah lahir dilakukan pengambilan sampel darah SHK. KN 2 hari ke 6, KN 3 hari ke 25. Perkembangan neonatus hingga usia 42 hari berlangsung fisiologis dengan terapi komplementer pijat bayi. Secara keseluruhan, asuhan kebidanan berkelanjutan berjalan normal sesuai standar sebagai upaya pemantauan dan deteksi dini komplikasi pada ibu dan bayi.
