Abstract

MIDWIFERY CARE FOR A 24 YEAR OLD MULTIGRAVID MOTHER "SD" FROM 20 WEEKS 6 DAYS OF PREGNANCY TO 42 DAYS OF POST-POSTAGE ABSTRAK Continuous midwifery care for multigravida pregnant women aims to provide early detection and prevention of complications in both mother and baby. Although multigravida mothers have previous experience with pregnancy and childbirth, complications may still occur during pregnancy, labor, and the postpartum period. Therefore, comprehensive monitoring is necessary to ensure the mother’s and fetus’s conditions remain normal and that the mother’s needs are optimally fulfilled. This study aimed to evaluate the implementation of comprehensive and continuous midwifery care (Continuity of Care) for Mrs. SD, a 24-year-old multigravida woman, starting from 20 weeks and 6 days of gestation until 42 days postpartum at UPTD Puskesmas Buleleng II, Buleleng Regency, from September 2025 to March 2026. The research method used was a case study with primary data collected through interviews, physical examinations, observations, and documentation, supported by secondary data from the Maternal and Child Health Book and medical records. During the second and third trimesters, the mother’s and fetus’s conditions remained physiological without complications. The mother routinely attended antenatal visits with standard 12T examinations and participated in pregnancy exercise programs. The labor process was normal without complications, supported by complementary therapies such as acupressure and massage to reduce pain. The baby was born healthy with a birth weight of 2900 grams. During the postpartum period, uterine involution, lochia discharge, and breast milk production progressed normally through regular monitoring from KF1 to KF4 according to midwifery care standards. Meskipun ibu multigravida telah memiliki pengalaman kehamilan dan persalinan sebelumnya, risiko komplikasi selama kehamilan, persalinan, maupun masa nifas tetap dapat terjadi. Oleh karena itu, pemantauan secara komprehensif diperlukan untuk memastikan kondisi ibu dan janin tetap normal serta kebutuhan ibu terpenuhi secara optimal. Penelitian ini bertujuan mengevaluasi hasil penerapan asuhan kebidanan secara menyeluruh dan berkesinambungan (Continuity of Care) pada Ny. SD usia 24 tahun multigravida, dimulai sejak usia kehamilan 20 minggu 6 hari hingga 42 hari masa nifas di UPTD Puskesmas Buleleng II, Kabupaten Buleleng, selama periode September 2025 sampai Maret 2026. Metode penelitian yang digunakan adalah studi kasus dengan pengumpulan data primer melalui wawancara, pemeriksaan fisik, observasi, dan dokumentasi, serta didukung data sekunder dari Buku KIA dan rekam medis. Selama kehamilan trimester II hingga trimester III, kondisi ibu dan janin berlangsung fisiologis tanpa komplikasi, serta ibu rutin melakukan kunjungan antenatal dengan pemeriksaan standar 12T dan mengikuti senam hamil. Proses persalinan berlangsung normal tanpa penyulit dengan bantuan terapi komplementer berupa akupresur dan pijat untuk mengurangi nyeri. Bayi lahir sehat dengan berat badan 2900 gram. Pada masa nifas, involusi uterus, pengeluaran lokia, dan produksi ASI berlangsung normal melalui pemantauan KF1 hingga KF4 sesuai standar pelayanan kebidanan.
