Abstract

Maternal and neonatal health remain major issues in healthcare services, as indicated by the high Maternal Mortality Rate (MMR) and Infant Mortality Rate (IMR). In Indonesia, maternal deaths were recorded at 4,005 cases in 2022, increasing to 4,129 cases in 2023, and reaching 4,151 cases by the first semester of 2024. Meanwhile, infant deaths in 2024 reached 33,131 cases, with 80.46% occurring during the neonatal period (0–7 days). In Bali Province, the MMR reached 107.2 per 100,000 live births and the IMR was 11.1 per 1,000 live births. These conditions highlight the importance of continuous midwifery care through the Continuity of Care (CoC) approach. This study aims to describe the implementation of comprehensive midwifery care for Mrs. “KS”, a 26-year-old multigravida, from 18 weeks and 6 days of gestation until 42 days postpartum. The method used was a case study with data collected through interviews, observation, physical examination, and documentation. The results showed that pregnancy, childbirth, postpartum, and newborn care were physiological without complications. Both mother and baby were in good condition, and the baby’s growth was optimal up to 42 days. In conclusion, the implementation of Continuity of Care midwifery services is effective in improving service quality and preventing complications in both mother and baby. Keywords: Continuity of Care; Pregnancy; Childbirth; Postpartum; Newborn   Kesehatan ibu dan bayi masih menjadi masalah utama dalam pelayanan kesehatan yang ditandai dengan tingginya Angka Kematian Ibu (AKI) dan Angka Kematian Bayi (AKB). Di Indonesia, AKI tahun 2022 sebesar 4.005 kasus meningkat menjadi 4.129 kasus pada tahun 2023 dan hingga semester I tahun 2024 mencapai 4.151 kasus. AKB tahun 2024 tercatat 33.131 kasus, dengan 80,46% terjadi pada masa neonatal usia 0–7 hari. Di Provinsi Bali, AKI mencapai 107,2 per 100.000 kelahiran hidup dan AKB sebesar 11,1 per 1.000 kelahiran hidup. Kondisi ini menunjukkan pentingnya pelayanan kebidanan yang berkesinambungan melalui pendekatan Continuity of Care (CoC). Tujuan penulisan ini adalah mengetahui gambaran penerapan asuhan kebidanan secara komprehensif pada Ny. “KS” umur 26 tahun multigravida dari usia kehamilan 18 minggu 6 hari hingga 42 hari masa nifas. Metode yang digunakan adalah studi kasus dengan pengumpulan data melalui wawancara, observasi, pemeriksaan fisik, dan dokumentasi. Hasil menunjukkan bahwa asuhan kehamilan, persalinan, nifas, dan bayi baru lahir berlangsung fisiologis tanpa komplikasi. Ibu dan bayi dalam kondisi sehat serta pertumbuhan bayi optimal hingga usia 42 hari. Simpulan menunjukkan bahwa penerapan asuhan kebidanan Continuity of Care efektif dalam meningkatkan kualitas pelayanan serta mencegah komplikasi pada ibu dan bayi.
