Abstract

NURSING CARE FOR Mr. S WITH INEFFECTIVE AIRWAY CLEARANCE DUE TO COPD IN THE KECAK WARD OF MANGUSADA REGIONAL GENERAL HOSPITAL IN 2026 ABSTRACT Chronic Obstructive Pulmonary Disease (COPD) is characterized by airflow limitation that is not fully reversible due to exposure to harmful particles or gases, such as cigarette smoke. COPD is one of the leading causes of death worldwide and often results in nursing problems, particularly ineffective airway clearance due to the accumulation of secretions. This case report aims to describe the implementation of nursing care for Mr. S with ineffective airway clearance due to COPD in the Kecak Ward of Mangusada Regional Hospital in 2026. The method used is descriptive with a case study approach conducted over five days.The assessment results showed that a 74-year-old male patient complained of shortness of breath, productive cough with sputum that was difficult to expectorate, and dyspnea that worsened when lying down and improved when sitting. Objective data the patient appears to have a cough with phlegm but the sputum is difficult to expel, excessive sputum production, the presence of additional breath sounds (rhonchi), the patient appearing restless, increased respiratory rate, and a rapid, shallow breathing pattern. The nursing diagnosis was ineffective airway clearance related to airway hypersecretion. Interventions included airway management, effective coughing exercises, respiratory monitoring. Evaluation showed that the patient’s condition improved, indicated by reduced shortness of breath, more effective coughing, easier sputum expectoration, and a return of respiratory rate to normal. S DENGAN BERSIHAN JALAN NAPAS TIDAK EFEKTIF AKIBAT PPOK DI RUANG KECAK RSD MANGUSADATAHUN 2026 ABSTRAK Penyakit Paru Obstruktif Kronik (PPOK) ditandai dengan hambatan aliran udara yang tidak sepenuhnya reversibel akibat paparan partikel atau gas berbahaya, seperti asap rokok. PPOK merupakan salah satu penyebab utama kematian di dunia dan sering menimbulkan masalah keperawatan berupa bersihan jalan napas tidak efektif akibat penumpukan sekret. Laporan kasus ini bertujuan untuk menggambarkan pelaksanaan asuhan keperawatan pada Tn. S dengan masalah bersihan jalan napas tidak efektif akibat PPOK di Ruang Kecak RSD Mangusada tahun 2026. Metode yang digunakan adalah deskriptif dengan pendekatan studi kasus selama lima hari. Hasil pengkajian menunjukkan pasien laki-laki usia 74 tahun mengeluh sesak napas, batuk berdahak yang sulit dikeluarkan, serta sesak yang memberat saat berbaring dan berkurang saat duduk. Data objektif menunjukkan pasien tampak mengalami batuk berdahak namun sputum sulit dikeluarkan, sputum berlebih, adanya bunyi napas tambahan ronkhi, pasien tampak gelisah, frekuensi napas meningkat, pola napas cepat dan dangkal. Diagnosis keperawatan adalah bersihan jalan napas tidak efektif berhubungan dengan hipersekresi jalan napas. Intervens meliputi manajemen jalan napas, latihan batuk efektif, dan pemantauan respirasi. Evaluasi menunjukkan kondisi pasien membaik, ditandai penurunan sesak, batuk lebih efektif, sputum mudah dikeluarkan, dan frekuensi napas kembali normal.
