Abstract

NURSING CARE FOR MRS. S WITH INEFFECTIVE PERIPHERAL PERFUSION DUE TO TYPE 2 DIABETES MELLITUS (Case Report in the Working Area of the Dawan I Community Health Center UPTD in 2026) ABSTRACT Ineffective peripheral perfusion is a condition characterized by decreased blood flow to peripheral tissues, commonly occurring in patients with Type 2 Diabetes Mellitus due to circulatory disorders. The prevalence ranges from 10% to 42% and may increase the risk of tissue damage in the lower extremities. This case report aims to describe the implementation of nursing care in a patient with ineffective peripheral perfusion due to Type 2 Diabetes Mellitus. The method used is a case study approach following the nursing process, including assessment, diagnosis, planning, implementation, and evaluation. The subject was Mrs. S, who complained of tingling in both feet. Assessment findings showed capillary refill time (CRT) >3 seconds, weak peripheral pulses, cold extremities, pale skin, decreased skin turgor, and edema in the lower extremities. Interventions were carried out over 4 days, including peripheral circulation monitoring, foot care education once daily for 15–20 minutes, and diabetic foot exercises once daily for 15–30 minutes. The implementation results showed gradual improvement, indicated by reduced tingling, improved CRT to <2 seconds, stronger peripheral pulses, more normal skin color, and decreased edema. The evaluation indicated partial to optimal improvement in peripheral perfusion, and the patient was able to perform the exercises independently. It is recommended that patients perform diabetic foot exercises regularly, maintain proper foot care, and monitor blood glucose levels to prevent further complications. S DENGAN PERFUSI PERIFER TIDAK EFEKTIF AKIBAT DIABETES MELLITUS TIPE 2 (Laporan Kasus di Wilayah Kerja UPTD Puskesmas Dawan I Tahun 2026) ABSTRAK Perfusi perifer tidak efektif merupakan kondisi menurunnya aliran darah ke jaringan perifer yang sering terjadi pada pasien Diabetes Mellitus tipe 2 akibat gangguan sirkulasi. Prevalensi gangguan ini berkisar 10%–42% dan dapat meningkatkan risiko kerusakan jaringan ekstremitas bawah. Laporan kasus ini bertujuan menggambarkan penerapan asuhan keperawatan pada pasien dengan perfusi perifer tidak efektif akibat Diabetes Mellitus tipe 2. Metode yang digunakan adalah studi kasus dengan pendekatan proses keperawatan meliputi pengkajian, diagnosis, perencanaan, implementasi, dan evaluasi. Subjek adalah Ny. S dengan keluhan kesemutan pada kedua kaki. Hasil pengkajian menunjukkan capillary refill time (CRT) >3 detik, nadi perifer lemah, akral dingin, warna kulit pucat, turgor kulit menurun, serta edema pada ekstremitas bawah. Intervensi dilakukan selama 4 hari, meliputi pemantauan sirkulasi perifer, edukasi perawatan kaki 1 kali/hari selama 15–20 menit, serta senam kaki diabetik 1 kali/hari selama 15–30 menit. Hasil implementasi menunjukkan perbaikan bertahap berupa berkurangnya kesemutan, CRT membaik hingga <2 detik, nadi perifer lebih kuat, warna kulit lebih normal, dan edema menurun. Evaluasi menunjukkan perfusi perifer membaik sebagian hingga optimal, serta pasien mampu melakukan senam kaki secara mandiri. Disarankan pasien melakukan senam kaki secara rutin, menjaga perawatan kaki, serta melakukan kontrol gula darah secara berkala untuk mencegah komplikasi lebih lanjut.
