Abstract
ABSTRAK Persalinan preterm, yaitu kelahiran sebelum usia kehamilan 37 minggu, merupakan salah satu penyebab utama morbiditas dan mortalitas neonatal di dunia. Persalinan ini dapat dipengaruhi oleh berbagai faktor, termasuk faktor obstetri dan sosial demografi. Penelitian ini bertujuan untuk mengetahui faktor-faktor yang berhubungan dengan persalinan preterm di RSUD Kabupaten Buleleng. Penelitian ini menggunakan desain case control dengan pendekatan retrospektif yang dilakukan pada Januari–Maret 2025. Populasi terjangkau adalah seluruh persalinan preterm di RSUD Kabupaten Buleleng selama Januari–Desember 2024. Sampel diambil secara simple random sampling, sebanyak 134 responden (kelompok kasus dan kontrol). Hasil analisis menunjukkan bahwa terdapat hubungan signifikan antara usia ibu dan persalinan preterm (p < 0,001; OR 7,305; 95% CI 3,597–14,838), status pekerjaan (p = 0,007), paritas (p = 0,002), ketuban pecah dini (p < 0,001; OR 6,508; 95% CI 2,247–18,851), dan partus prematurus iminens (p < 0,001; OR 4,087; 95% CI 2,129–7,843). Tidak ditemukan hubungan signifikan antara tingkat pendidikan dan riwayat preeklamsia terhadap persalinan preterm. Faktor usia ibu merupakan prediktor dominan. Hasil ini menunjukkan pentingnya deteksi dini oleh tenaga kesehatan untuk mengurangi risiko kelahiran prematur dan komplikasinya. Kata kunci: case control, faktor obstetri, faktor sosialdemografi, persalinan preterm ABSTRACT Preterm birth, defined as delivery before 37 completed weeks of gestation, is one of the leading causes of neonatal morbidity and mortality worldwide. This condition can be influenced by various factors, including obstetric and sociodemographic factors. This study aimed to identify the factors associated with preterm birth at the Regional Public Hospital (RSUD) of Buleleng Regency. A case-control study design with a retrospective approach was employed from January to March 2025. The accessible population included all preterm births at RSUD Buleleng from January to December 2024. A total of 134 respondents (case and control groups) were selected using simple random sampling. The results showed significant associations between maternal age and preterm birth (p < 0.001; OR 7.305; 95% CI 3.597–14.838), employment status (p = 0.007), parity (p = 0.002), premature rupture of membranes (p < 0.001; OR 6.508; 95% CI 2.247–18.851), and threatened preterm labor (p < 0.001; OR 4.087; 95% CI 2.129–7.843). No significant association was found between educational level or history of preeclampsia and preterm birth. Maternal age was identified as the most dominant predictor. These findings highlight the importance of early detection by healthcare providers to prevent preterm birth and its associated complications.
