Abstract

NURSING CARE FOR NY S WITH INEFFECTIVE PERIPHERAL PERFUSION DUE TO TYPE II DIABETES MELLITUS IN KASUARI ROOM RSUD BALI MANDARA ABSTRACT Type II diabetes mellitus is a chronic metabolic disease that may lead to serious complications, including ineffective peripheral perfusion. One of the recommended therapies to improve peripheral circulation is foot exercise. This case study aims to describe the nursing care provided to Mrs. S, a patient with ineffective peripheral perfusion due to type II diabetes mellitus at Kaswari Ward, Bali Mandara General Hospital. The research used a descriptive method with a case study approach. Data were collected through interviews, observations, physical examinations, and Ankle Brachial Index (ABI) measurements. The initial assessment revealed that the patient experienced numbness, tingling, cold extremities, pale skin, weak peripheral pulses, and an ABI value of 0.86 mmHg. The nursing intervention included peripheral circulation care and daily diabetic foot exercises over five consecutive days. After the intervention, the patient’s ABI improved to 1 mmHg, and she no longer experienced numbness. This case concludes that foot exercises are effective in improving peripheral perfusion in patients with type II diabetes mellitus. Patients are encouraged to continue regular foot exercises as part of their self-care routine. ASUHAN KEPERAWATAN PADA NY S DENGAN PERFUSI PARIFER TIDAK EFEKTIF AKIBAT DIABETES MELITUS TIPE II DIRUANG KASUARI RSUD BALI MANDARA ABSTRAK Diabetes melitus tipe II merupakan salah satu penyakit metabolik kronis yang dapat menimbulkan komplikasi serius, termasuk gangguan perfusi perifer. Salah satu terapi yang dapat digunakan untuk meningkatkan sirkulasi perifer adalah senam kaki. Penelitian ini bertujuan untuk menggambarkan asuhan keperawatan pada pasien Ny. S dengan masalah perfusi perifer tidak efektif akibat diabetes melitus tipe II yang dirawat di ruang Kaswari RSUD Bali Mandara. Desain penelitian ini menggunakan pendekatan deskriptif dengan metode studi kasus. Data diperoleh melalui wawancara, observasi, dan pemeriksaan fisik serta penunjang, seperti pengukuran nilai Ankle Brachial Index (ABI). Hasil pengkajian menunjukkan Ny. S mengalami kesemutan, mati rasa, akral teraba dingin, kulit pucat, nadi perifer lemah, dan nilai ABI 0,86 mmHg. Intervensi yang diberikan berupa perawatan sirkulasi dan senam kaki selama lima hari berturut-turut. Setelah intervensi, terjadi peningkatan nilai ABI menjadi 1 mmHg dan subjek tidak lagi mengeluhkan kesemutan. Kesimpulan dari studi kasus ini menunjukkan bahwa senam kaki efektif dalam meningkatkan perfusi perifer pada pasien diabetes melitus tipe II. Diharapkan pasien melanjutkan latihan senam kaki secara rutin sebagai bagian dari perawatan mandiri.
