Abstract

RELATIONSHIP BETWEEN HbA1c LEVELS AND DIABETIC PERIPHERAL NEUROPATHY IN TYPE II DIABETES MELLITUS PATIENTS AT RSUD KLUNGKUNG IN 2025 ABSTRACT The number of type II diabetes mellitus cases with complications at RSUD Klungkung in 2023-2024 reached 102 cases. Diabetic peripheral neuropathy is one of the complications of diabetes mellitus that alters the function of sensory, motor, and autonomic nerves. This study aims to determine the relationship between HbA1c levels and diabetic peripheral neuropathy in patients with type II diabetes mellitus at RSUD Klungkung. The research design is a correlational analytic with a retrospective cohort approach. The research sample consisted of type II diabetes mellitus patients who met the inclusion criteria, totaling 96 respondents, selected using the consecutive sampling technique. The research results show that 52.1% of respondents are in the age range of 56-65 years, 80.2% have suffered from type II diabetes mellitus for 5-10 years, the majority 72.9% have HbA1c levels ≥7%, and 68.8% have positive diabetic peripheral neuropathy. As many as 59.4% of respondents with uncontrolled HbA1c levels and 9.4% with controlled HbA1c levels experienced diabetic peripheral neuropathy. The results of the contingency coefficient test obtained a value (r) of 0.409 and a p-value of 0.001 (α < 0.05), indicating a significant positive relationship with moderate strength between HbA1c levels and diabetic peripheral neuropathy. Based on this, it is recommended that health practitioners at RSUD Klungkung provide counseling on glycemic control and screening for type II diabetes mellitus patients regarding the symptoms of diabetic peripheral neuropathy. Neuropati perifer diabetik menjadi salah satu komplikasi pada diabetes melitus yang mengubah fungsi saraf sensorik, motorik, otonom. Penelitian ini bertujuan untuk mengetahui hubungan kadar HbA1c dengan neuropati perifer diabetik pada pasien diabetes melitus tipe II di RSUD Klungkung. Desain penelitian adalah analitik korelasional dengan pendekatan kohort retrospektif. Sampel penelitian yaitu pasien diabetes melitus tipe II yang memenuhi kriteria inklusi sebanyak 96 responden dengan teknik consecutive sampling. Hasil penelitian menunjukkan sebanyak 52,1% responden pada rentang usia 56-65 tahun, sebanyak 80,2% menderita diabetes melitus tipe II selama 5-10 tahun, sebagian besar 72,9% memiliki kadar HbA1c ≥7% dan 68,8% mengalami positif neuropati perifer diabetik. Sebanyak 59,4% responden dengan kadar HbA1c tidak terkontrol dan 9,4% dengan kadar HbA1c terkontrol mengalami neuropati perifer diabetik. Hasil uji koefisien kontingensi diperoleh nilai (r) sebesar 0,409 dan p-value 0,001 (α < 0,05) sehingga dapat dinyatakan terdapat hubungan positif dengan kekuatan sedang yang bersignifikan antara kadar HbA1c dengan neuropati perifer diabetik. Berdasarkan hal tersebut, disarankan kepada praktisi kesehatan RSUD Klungkung dapat memberikan penyuluhan mengenai kontrol glikemik serta skrinning kepada pasien diabetes melitus tipe II mengenai gejala neuropati perifer diabetik.
