Abstract

ABSTRACT Mothers and children are family members who need to be prioritized in the implementation of health efforts, midwives in providing care are carried out with continuous midwifery services through Continuity of Care (CoC). This final report aims to determine the results of the implementation of midwifery care for Mother "PWH" aged 26 years multigravida from 18 weeks 6 days of pregnancy to 42 days of postpartum period and her child who received comprehensive and continuous continuity of care midwifery care. Midwifery care started from October 2024 to March 2025. The results of the care provided to patients during the pregnancy process until 42 days of postpartum period took place physiologically and were in accordance with midwifery care standards, including assessment, problem formulation, planning, implementation, evaluation and recording of midwifery care. The standard of midwifery care services during pregnancy uses 12 T and delivery planning, prevention of complications (P4K). The spontaneous labor process with care for the first-fourth stage with APN standards, there were no complications where changes in involution, lochia discharge, and lactation were still monitored by conducting 4 postpartum visits, 3 visits to the baby. Based on the results of the midwifery care that has been provided, it can be concluded that the implementation of midwifery care for Mother "PWH" starting from pregnancy, childbirth, newborns, and postpartum has been given care according to standards and takes place physiologically.
