Abstract

IMPLEMENTASI PENGATURAN POSISI SEMI FOWLER PADA PASIEN DENGAN POLA NAPAS TIDAK EFEKTIF AKIBAT GAGAL GINJAL KRONIS DI RSUD KLUNGKUNG ABSTRAK Gagal ginjal kronis adalah penurunan fungsi ginjal menahun dan berkelanjutan yang mengakibatkan kerusakan jaringan ginjal yang progresif dan ireversibel dengan penurunan laju filtrasi glomerulus (LFG) yang kurang dari 60 ml/menit/1,73 m2 selama 3 bulan. Masalah keperawatan yang muncul pada pasien gagal ginjal kronis yaitu pola napas tidak efektif. Penelitian ini bertujuan untuk mengetahui implementasi pengaturan posisi semi fowler pada pasien dengan pola napas tidak efektif akibat gagal ginjal kronis di RSUD Klungkung. Jenis penelitian ini adalah deskriptif dengan rancangan studi kasus. Jumlah subjek penelitian sebanyak satu orang pasien. Diagnosis keperawatan yaitu Pola napas tidak efektif berhubungan dengan hambatan upaya napas dibuktikan dengan pasien mengeluh sesak napas sejak kemarin malam, pasien mengeluh sesak bertambah saat berbaring telentang, tampak penggunaan otot bantu pernapasan, tampak adanya fase ekspirasi memanjang, tampak pernapasan pursed-lip, tampak pernapasan cuping hidung, RR 26x/menit (takipnea), dan SpO2 94%. Posisi semi fowler diberikan selama 30 menit. Evaluasi didapatkan sesak napas menurun, sesak napas saat berbaring telentang menurun, penggunaan otot bantu napas menurun, pemanjangan fase ekspirasi menurun, frekuensi napas membaik, kedalaman napas membaik, pernapasan pursed-lip menurun dan pernapasan cuping hidung menurun. Kata Kunci: Gagal Ginjal Kronis, Pola Napas Tidak Efektif, Posisi Semi Fowler IMPLEMENTATION OF SEMI FOWLER POSITIONING IN PATIENTS WITH INEFFECTIVE BREATHING PATTERNS DUE TO CHRONIC RENAL FAILURE AT KLUNGKUNG HOSPITAL ABSTRACT Chronic renal failure is a chronic and sustained decline in kidney function that results in progressive and irreversible damage to kidney tissue with a decrease in glomerular filtration rate (LFG) of less than 60 ml/min/1.73 m2 for 3 months. Nursing problems that arise in patients with chronic renal failure are ineffective breathing patterns. This study aims to determine the implementation of semi-fowler positioning in patients with ineffective breathing patterns due to chronic renal failure at Klungkung Hospital. This type of research is descriptive with a case study design. The number of research subjects was one patient. The nursing diagnosis, namely ineffective breathing patterns associated with airway obstruction, is evidenced by the patient complaining of shortness of breath since last night, the patient complains of increasing shortness of breath when lying on his back, the use of respiratory muscles is seen, the expiratory phase is elongated, the patient's breathing pattern is tachypnea with RR 26x/min and SpO2 94%, pursed-lip breathing appears and nasal lobe breathing appears. The semi-fowler position was given for 30 minutes. The evaluation found that shortness of breath decreased, shortness of breath when lying on the back decreased, the use of breathing muscles decreased, the lengthening of the expiratory phase decreased, the frequency of breathing improved, the depth of breath improved, pursed-lip breathing decreased and nasal lobe breathing decreased.
