Abstract

Sectio caesarea merupakan metode persalinan yang dilakukan dengan menggunakan metode pembedahan yang dapat menimbulkan masalah pada ibu yaitu produksi ASI dalam jumlah minimal. Tujuan dari studi kasus adalah untuk mengetahui gambaran asuhan keperawatan menyusui tidak efektif pada ibu post sectio caesarea dengan pemberian intervensi inovasi massage woolwich. Massage woolwich adalah pijat yang dilakukan di laktiferus daerah sinus 1-1,5 cm diatas areola. Pada tahap pengkajian ditemukan data subjektif ibu mengatakan payudara tegang, bayinya tidak mau melekat ketika disusui, data objektif bayi tampak tidak mau melekat pada payudara ibu, bayi tampak menangis ketika disusui. Diagnosis keperawatan yang diangkat yaitu menyusui tidak efektif berhubungan dengan kurangnya perlekatan bayi pada payudara dibuktikan dengan ibu belum percaya diri dalam menyusui, bayi tidak mampu melekat pada payudara, bayi menangis ketika disusui, tampak ada pengeluaran colostrum. Penyusunan rencana keperawatan dilakukan dengan mengacu pada SLKI yaitu status menyusui, intervensi keperawatan disusun dengan menggunakan SIKI : edukasi menyusui dan konseling laktasi. Proses implementasi dilakukan 3x24 jam dengan menerapkan rencana keperawatan yang telah disusun serta menerapkan massage woolwich. Hasil evaluasi keperawatan menunjukan terdapat perbaikan status menyusui pasien Hasil studi kasus menunjukan penerapan massage woolwich mampu dalam meningkatkan produksi ASI pada ibu post sectio caesare. Kata kunci : Sectio caesare, Menyusui tidak efektif, Massage woolwich Sectio caesarea is a method of delivery that is carried out using a surgical method that can cause problems for the mother, namely the production of minimal amounts of breast milk. The aim of the case study is to find out the picture of ineffective breastfeeding nursing care for mothers post caesarean section by providing the Woolwich massage innovation intervention. Woolwich massage is a massage performed on the lactiferous sinus area 1-1.5 cm above the areola. At the assessment stage, it was found that the mother's subjective data stated that her breasts were tense, the baby did not want to latch on when breastfed, objective data the baby did not seem to want to latch on to the mother's breast, the baby appeared to cry when breastfed. The nursing diagnosis raised was ineffective breastfeeding related to the baby's lack of attachment to the breast as evidenced by the mother not being confident in breastfeeding, the baby being unable to latch onto the breast, the baby crying when breastfed, and there was visible colostrum discharge. The preparation of nursing plans is carried out by referring to SLKI, namely breastfeeding status, nursing interventions are prepared using SIKI: breastfeeding education and lactation counseling. The implementation process is carried out 3x24 hours by implementing the nursing plan that has been prepared and implementing Woolwich massage. The results of the nursing evaluation showed that there was an improvement in the patient's breastfeeding status. The results of the case study showed that the application of Woolwich massage was able to increase breast milk production in post-cesarean section mothers.
