Abstract

NURSING CARE FOR INEFFECTIVE PERIPHERAL PERFUSION WITH BUERGER ALLEN EXERCISE INTERVENTION IN PATIENTS WITH TYPE 2 DIABETES MELLITUS IN THE RSUD WANGAYA ABSTRACT Diabetes mellitus is a chronic metabolic disorder caused by the pancreas that cannot produce enough insulin or the body is ineffective in using the insulin produced. In patients with diabetes mellitus, ineffective peripheral perfusion occurs due to instability of glucose levels in the blood. One of the management of ineffective peripheral perfusion in a non-pharmacological way is doing Buerger Allen Exercise (BAE). The aim is to determine the nursing care of ineffective peripheral perfusion with buerger allen exercise intervention in patients with type 2 diabetes mellitus in the cendrawasih room of Wangaya Hospital. The method used in this research is a case study. The results of the assessment of Mr. A is complaining of weakness, headache, and often feels tingling in both legs, the patient complains of pain in the legs on the lower extremities palpated weaker, decreased leg skin turgor, there is little edema on both lower extremities, there is udema on the feet, the skin color on the feet is pale, the acral feels cold, the ABI value is The intervention given is buerger allen exercise which is given 3x24 hours for ± 15 minutes. Implementation is carried out in accordance with the interventions prepared and carried out 3x24 hours with the provision of buerger allen exercise as the main action. The nursing evaluation obtained was an increase in ABI value to 0.91 with complaints of more comfort in the legs, as long as BAE was not tingling, pain in the legs had also decreased. Legs feel lighter to move. Conclusion: The intervention of buerger allen exercise as an innovative intervention can improve circulation to the peripheral areas of patients with diabetes mellitus characterized by an increase in ABI values and an increase in the patient's peripheral pulse pressure. Suggestion: It is hoped that buerger allen exercise therapy will be an alternative in improving peripheral circulation in patients with diabetes mellitus. Keywords: buerger allen exercise, type 2 diabetes mellitus, ineffective peripheral perfusion ASUHAN KEPERAWATAN PERFUSI PERIFER TIDAK EFEKTIF DENGAN INTERVENSI BUERGER ALLEN EXERCISE PADA PASIEN DIABETES MELITUS TIPE 2 DI RSUD WANGAYA ABSTRAK Diabetes melitus adalah gangguan metabolik menahun diakibatkan oleh pankreas yang tidak dapat memproduksi cukup insulin ataupun tidak efektifnya tubuh dalam menggunakan insulin yang diproduksi. Pada pasien diabetes mellitus, perfusi perifer tidak efektif terjadi karena ketidakstabilan kadar glukosa dalam darah. Salah satu penatalaksanaan perfusi perifer tidak efektif dengan cara non farmakologis yaitu melakukan Buerger Allen Exercise (BAE). Tujuannya adalah untuk mengetahui asuhan keperawatan perfusiperifer tidak efektif dengan intervensi buerger allen exercise pada pasien diabetes melitus tipe 2 di ruang cendrawasih RSUD Wangaya. Metode yang digunakan dalam penelitian ini yaitu studi kasus. Hasil pengkajian terhadap Tn. A yaitu mengeluh , sakit kepala, serta sering terasa kesemutan pada kedua kakinya, pasien mengeluh nyeri pada kaki pada ekstremitas bawah teraba lebih lemah, turgor kulit kaki menurun, terdapat sedikit edema pada kedua ekstremitas bawah, terdapat udema pada kaki, warna kulit pada kaki pucat, akral teraba dingin, nilai ABI 0,81. Intervensi yang diberikan yaitu buerger allen exercise yang diberikan 3x24 jam selama ±15 menit. Implementasi dilakukan sesuai dengan intervensi yang disusun dan dilakukan 3x24 jam dengan pemberian buerger allen exercise sebagai tindakan utama. Evaluasi keperawatan yang peroleh yaitu peningkatan nilai ABI menjadi 0,91 dengan keluhan lebih nyaman pada kaki, selama dilakukan BAE tidak kesemutan, nyeri pada kaki juga sudah berkurang. Kaki merasa lebih ringan untuk digerakkan. Simpulan : Intervensi buerger allen exercise sebagai intervensi inovasi dapat meningkatkan sirkulasi ke daerah perifer pasien dengan diabetes melitus yang ditandai dengan peningkatan nilai ABI dan peningkatan tekanan nadi perifer pasien. Saran : Diharapkan terapi buerger allen exercise menjadi salah satu alternatif dalam meningkatkan sirkulasi perifer pada pasien DM..
