Abstract

Nursing Care for Skin Integrity Disorders in Diabetic Foot Patients with Moist Wound Dressing Treatment in the Cendrawasih Room of Wangaya Hospital ABSTRACT Diabetic foot is a chronic complication of diabetes mellitus in the form of chronic wounds on the feet caused by peripheral neuropathy, peripheral arterial disease, or a combination of these symptoms. Diabetic feet can increase morbidity, mortality, and reduce the patient's quality of life. The aim of this research is to determine nursing care for impaired skin integrity in patients who experience diabetic foot in Cendrawasih Room at Wangaya Regional Hospital. The results of the research in the nursing assessment showed skin layer damage on heel of the patient's right foot, a 1.5 cm wide wound showed the dermis layer around the wound, there was bleeding. The nursing diagnosis formulated was impaired skin integrity related to peripheral neuropathy. The nursing plan refers to SLKI with the outcome of increased skin and tissue integrity and SIKI with the main interventions for skin integrity care and wound care as well as wound care innovation interventionsmoist wound dressing given first thing in the morning. Nursing implementation is carried out in accordance with the nursing plan that has been previously planned for 3 x 24 hours. Nursing evaluation showed that the goal was partially resolved by reducing bleeding and necrosis, increasing hydration, and reducing the wound score from 40 to 30. The conclusion was that providing wound caremoist wound dressing can be effectively given for wound healing in patients with diabetic foot. Keywords: diabetic foot, skin and tissue integrity disorders, moist wound dressing ASUHAN KEPERAWATAN GANGGUAN INTEGRITAS KULIT PADA PASIEN DIABETIC FOOT DENGAN PERAWATAN LUKA MOIST WOUND DRESSING DI RUANG CENDRAWASIH RSUD WANGAYA ABSTRAK Kaki diabetes adalah komplikasi kronik diabetes melitus berupa luka kronik pada kaki disebabkan oleh neuropati perifer, penyakit arteri perifer, maupun kombinasi dari gejala tersebut. Kaki diabetes dapat meningkatkan morbiditas, mortalitas, dan mengurangi kualitas hidup pasien. Tujuan dari penelitian ini untuk mengetahui asuhan keperawatan gangguan integritas kulit/jaringan pada pasien yang mengalami diabetic foot di Ruang Cendrawasih RSUD Wangaya. Hasil penelitian pada pengkajian keperawatan terdapat kerusakan jaringan dan lapisan kulit pada tumit kaki kanan pasien, luka seluas 1,5 cm terlihat lapisan dermis di sekitar luka, terdapat perdarahan. Diagnosis keperawatan yang dirumuskan adalah gangguan integritas kulit/jaringan berhubungan dengan neuropati perifer. Rencana keperawatan mengacu pada SLKI dengan luaran integritas kulit dan jaringan meningkat dan SIKI dengan intervensi utama perawatan integritas kulit dan perawatan luka serta intervensi inovasi perawatan luka moist wound dressing diberikan pagi hari sekali. Implementasi keperawatan dilakukan sesuai dengan rencana keperawatan yang sudah direncanakan sebelumnya selama 3 x 24 jam. Evalusasi keperawatan menunjukkan tujuan teratasi sebagian dengan mengalami penurunan perdarahan dan nekrosis, peningkatan hidrasi, serta terjadi penurunan skor luka dari 40 menjadi 30. Kesimpulannya bahwa pemberian perawatan luka moist wound dressing dapat efektif diberikan untuk penyembuhan luka pada pasien dengan diabetic foot.
