Abstract

RELATIONSHIP BETWEEN THE DURATION OF DIABETES MELLITUS WITH DIABETIC PERIPHERAL NEUROPATHY IN TYPE 2 DIABETES MELLITUS PATIENTS AT DENPASAR II PUBLIC HEALTH CENTER 2023 ABSTRACT Type 2 diabetes mellitus is characterized by insulin resistance due to body cells not being able to use the insulin they produce effectively, causing an increase in blood glucose levels. If in the long term high blood glucose levels are not controlled can cause complications, one of which is peripheral diabetic neuropathy. Diabetic neuropathy is a chronic complication that results in damage to the nerves. Efforts that can be made are controlling blood glucose levels, doing early detection of neuropathy, and doing foot care. The purpose of this study to determine the duration of diabetes mellitus with diabetic peripheral neuropathy in type 2 diabetes mellitus patients. The type of research used is non-experimental with correlational design and cros sectional approach. The number of samples is 80 people and used non probability sampling with purposive sampling. The duration diabetes mellitus was measured by the observation sheet and diabetic peripheral neuropathy was measured by the DNS questionnaire. The results showed (52.5%) aged 46-63 years, female (65.0%), (72.5%) had peripheral diabetic neuropathy and the duration of type 2 diabetes mellitus > 5 years (86.3%). Test the hypothesis used regresi logistic with p value of the Omnibus test = 0.006 (α = 0.05) and the Nagelkerke R Squere value = 0.128. The study found significant relationship between the duration of diabetes mellitus with diabetic peripheral neuropathy in type 2 diabetes mellitus patients. The nurse need to do early detection with regular foot examinations to find out any signs and symptoms of nerve damage, so as to prevent more severe complications of DM. Keywords : duration of DM, diabetic peripheral neuropathy, type 2 DM vii HUBUNGAN LAMA MENDERITA DIABETES MELITUS DENGAN NEUROPATI PERIFER DIABETIK PADA PASIEN DIABETES MELITUS TIPE 2 DI UPTD PUSKESMAS II DENPASAR BARAT TAHUN 2023 ABSTRAK Diabetes melitus tipe 2 ditandai dengan terjadinya resistensi insulin akibat sel tubuh tidak mampu menggunakan insulin yang diproduksinya secara efektif, sehingga menyebabkan terjadinya peningkatan kadar glukosa darah. Apabila dalam jangka panjang tingginya kadar glukosa darah yang tidak terkontrol dapat menimbulkan komplikasi salah satunya neuropati diabetik perifer. Neuropati diabetik adalah komplikasi kronik yang mengakibatkan kerusakan pada saraf. Upaya yang dapat dilakukan yaitu mengontrol kadar glukosa darah, melakukan deteksi dini neuropati, dan melakukan perawatan kaki. Tujuan penelitian ini untuk mengetahui hubungan lama menderita diabetes melitus dengan neuropati perifer diabetik pada pasien DM tipe 2. Jenis penelitian yang digunakan adalah non-eksperimen dengan rancangan korelasional dan pendekatan cros sectional. Jumlah sampel sebanyak 80 orang dengan metode non probability sampling dengan purposive sampling. Pengumpulan data menggunakan lembar observasi untuk variabel lama menderita DM dan variabel neuropati perifer diabetik menggunakan kuisioner DNS. Hasil penelitian menunjukkan (52.5%) berusia 46-63 tahun, berjenis kelamin perempuan (65.0%), (72,5%) mengalami neuropati diabetik perifer dan menderita diabetes melitus tipe 2 > 5 tahun (86,3%). Uji hipotesis menggunakan uji regresi logistic dengan nilai p omnibus sebesar 0,006 dan nilai nagelkerke R Square sebesar 0,128. Disimpulkan bahwa ada pengaruh signifikan antara lama menderita diabetes melitus dengan neuropati perifer diabetik pada pasien diabetes melitus tipe 2. Perawat perlu melakukan deteksi dini dengan pemeriksaan kaki secara berkala untuk mengetahui adanya tanda dan gejala kerusakan saraf, sehingga dapat mencegah komplikasi DM yang lebih berat.
